Adolescent Groups Application
Our Youth Program currently offers two adolescent groups (ages 13-17), one in-person (Sat 1:30pm) and one online (Thurs 5:30pm) to allow flexibility and convenience. re:MIND’s peer support group is a safe place for adolescents to talk about coping skills, process feelings, and provide support to their peers. Both groups are led by trained facilitators and are professionally monitored and managed. The group is best utilized as only one part of an overall wellness plan which may include therapy, medication, counseling, etc.
To apply for this group, a parent/legal guardian will need to complete the Adolescent Intake Form as well as the Parent/Guardian and Adolescent Consent Form that follows. The entire application may take approximately 15 minutes to complete. Please note that a small portion of the consent form needs to be reviewed with your adolescent.
Upon receiving both forms, the re:MIND Program staff will review application. The parent/legal guardian will then be notified by e-mail within 5 business days of initial screening. If application is accepted, group logistics will be provided. For the adolescent to attend group, the parent/legal guardian must be present in the building during group time. The adolescent and parent/legal guardian must also complete the weekly sign in sheet.
